■National University Hospitals Facing Crisis of Medical Collapse Due to "Largest Ever" Deficit

Expected deficit of 40 billion yen

On October 3, 2025, the National University Hospital Conference announced at a press conference that the deficit of national university hospitals is projected to be "6 billion yen in fiscal year 2023, 28.6 billion yen in fiscal year 2024, and over 40 billion yen in fiscal year 2025 (estimated)." Chairman Seiji Otori stated, "It is no exaggeration to say that we are facing the greatest crisis in history."

Only option is to reduce facility and equipment costs…

The cause of the deficit is increased expenditures: "Medical expenses = 25.8 billion yen increase, utility costs = 900 million yen increase, personnel costs = 32.5 billion yen increase, outsourcing costs, etc. = 8.9 billion yen increase" (compared to the previous year). Meanwhile, "facility and equipment costs" have decreased by 7.6 billion yen. According to Chairman Otori, "We cannot purchase new medical equipment. 98% of medical equipment cannot be updated."

Risk of inability to perform examinations

When asked about the impact on patients, Yasuhisa Fujii, Director of Tokyo University of Science Hospital, replied, "We are updating ventilators and ECMO (extracorporeal membrane oxygenation/artificial heart-lung machines). However, imaging diagnostic equipment such as CT scanners do not directly affect patients, so we use them for a long time. It wouldn't be surprising if a situation arises where we cannot perform patient examinations."

Japan Medical Association Chairman Yoshiro Matsumoto

Q.  Medical collapse is already occurring in rural areas, as well as in urban areas, isn't it? (Japan Medical Association Chairman Yoshiro Matsumoto) "Complete medical collapse is occurring. We are making management efforts, but due to soaring prices and rising personnel costs, management has become unsustainable, expenditures and expenses have ballooned, and all hospitals are experiencing significant losses."
